Before you fill out your Smart Online Store application, please ensure you have the complete documentary requirements for quick processing and approval.

  1. I own a credit card

    Please send scans or high resolution pictures of the following documents:

    • Your credit card statement of account for the last 2 months (first two pages)
    • Actual credit card (front only)
    • One government-issued ID (ex. driver's license, passport, etc.) or two secondary IDs (ex. senior citizen ID, company ID, etc.) Important: The names on your ID and credit card should match. Please make sure you send a front and back scan/picture of your ID.
    • If you want your billing address for this application different from the one in your credit card's statement of account, submit another proof of address (ex. utility bills, bank statement, etc.). If the proof of address is not under your name, get a Letter of Authority from the one whose name is indicated. Download sample of Letter of Authority

  2. I am employed but I don’t have a credit card

    Once your application has been processed, you may pay online via Bancnet ATM card (PesoPay); or via online banking or over-the-counter payment through partner banks or payment centers (Dragonpay).

    Please send scans or high resolution pictures of the following documents:

    • Company ID
    • One government-issued ID (ex. driver's license, passport, etc.) or another secondary ID (ex. senior citizen ID, sports club membership, etc.) Please make sure you send a front and back scan/picture of your ID.
    • Proof of address. Submit a billing statement bearing the address you want to be billed to (ex. utility bills, bank statement, etc.). If the proof of address is not under your name, get a Letter of Authority from the one whose name is indicated. Download sample of Letter of Authority
    • Your latest month's payslip. (Example: If today is May 2, the pay slips that you need to submit are your pay slips for April 1-15 and April 16-30.) If your job is commission-based, send us your pay slip for the last 3 months.

    Plus, either of the following:

    • Certificate of Employment stating your compensation package and tenure, issued within the last 30 days.

    OR

    • Latest Income Tax Return.

  3. I have my own business under Sole Proprietorship

    Once your application has been processed, you may pay online via credit card or Bancnet ATM card (PesoPay); or via online banking or over-the-counter payment through partner banks or payment centers (Dragonpay).

    Please send scans or high resolution pictures of the following documents:

    • One government-issued ID (ex. driver's license, passport, etc.) or two secondary IDs (ex. senior citizen ID, company ID, etc.) Important: The names on your ID and credit card should match. Please make sure you send a front and back scan/picture of your ID.
    • Business Permit or Mayor's Permit
    • Latest ITR 2317
    • Latest audited financial statement
    • Proof of address. Submit a billing statement bearing the address you want to be billed to (ex. utility bills, bank statement, etc.). If the proof of address is not under your name, get a Letter of Authority from the one whose name is indicated. Download sample of Letter of Authority

  4. I have an existing subscription with another Telecom company

    Once your application has been processed, you may pay online via credit card or Bancnet ATM card (PesoPay); or via online banking or over-the-counter payment through partner banks or payment centers (Dragonpay).

    You must have an individual account subscription with another telecom company in the past 6 months (company-paid mobile phone subscriptions are not acceptable).

    Please send scans or high resolution pictures of the following documents:

    • One government-issued ID (ex. driver's license, passport, etc.) or two secondary IDs (ex. senior citizen ID, company ID, etc.) Important: The names on your ID and credit card should match. Please make sure you send a front and back scan/picture of your ID.
    • Your latest billing statement from another telecom company.

      NOTE: Your statement of account should reflect that it is paid-in-full, OR if already due for payment, must accompany proof of payment (ex. official receipt)

    • Preferred billing address should match the address in your current telecom company's billing statement. Otherwise you would need to submit a proof of address bearing the address you want to be billed to (ex. utility bills, bank statement, etc.). If the proof of address is not under your name, get a Letter of Authority from the one whose name is indicated.
